PVUSD Athlete Participation Requirements

The following tasks are required and must be completed prior to practice

Step 1:

  •  Athletics utilizes an online registration tool for all student athletes. Parents will need to create a user account, complete forms, and upload them into Register My Athlete (RMA) prior to tryouts.  
  •  If you cannot remember your username, please contact the athletic secretary to get it, do not create another profile. 
  • Athletic Secretary email is staharris@pvschools.net

Step 2:

  •  To participate in any sport, you must first review all Electronic Documents and complete  E-signatures in Register My Athlete.  
  •  All athletes and their parents must also view the online Informed Consent Video  & Hazing Awareness Video  prior to participation. 

Step 3: To participate in any sport, you must complete and upload the following documents for the current school year: 

  1. The AIA Pre-participation Physical Evaluation and Examination Packet,
  2. The AIA Consent to Treat and
  3. The AIA Brain Injury/Concussion (MTBI) forms.  

  •  The Physical Packet requires a MD, DO, NP, or PA-C signature, and must be dated after March 1st of the participation school year.  
  •  These documents can be found in RMA or https://www.pvschools.net/programs/athletics/high-school-athletics/participate-in-athletics

 

All  athletes must also complete the Arizona Interscholastic Association's (AIA) required Concussion/Brain and Opioid Education Course and print out completed certificate documents.

  • First register as a student of your school before beginning the course.
  • Print out the completion certificate and upload in Register My Athlete
  •  Concussion/Brain Course  
  •  All completed documents must be legible, include all pages and be uploaded into Register My Athlete.   Your account must read “Ready for Tryouts” in order to try out for your chosen sport. 

Step 4: 

  •  Once students have made the final roster of their chosen sport, the Arizona Interscholastic Associates (AIA) participation fee is due by Friday, 11/27/26. All fees need to be paid to the school's bookstore or through the Webstore & Payment Portal.  Athletes pay a fee of $200 for AIA athletic participation in each sport. Families are capped at $400 each school year regardless of the number of individual athletic participation fees they may have. (This also includes Middle School Festival Sports, Speech & Debate and Robotics).  For students who require assistance with athletic fees, please contact the school.  
  •  Parents: For first-time login, you can access your student's account by entering the following information in the username and password fields. 

  1. Username: Student ID Number
  2. Password: first initial and last name

  •  The  webstore  accepts all major credit cards or you can go the bookstore from 7:15am-2:30pm, Monday-Friday.

Other Info:

  • All participants must have passed all classes from the previous quarter, or made up the same number of failed classes in summer school (summer school classes do not have to be the same as those failed).
  • All participants must insurance.  If you need to purchase insurance for your student, K&K Insurance is a possible option for low-cost insurance. You can apply online or pick up a packet from the Athletic Director's office.
  • If your student is transferring from another school, school district or state you must complete the AIA 550 Transfer Form. When transferring from a different state, quite often the school was not a member of the Arizona Interscholastic Association (AIA), therefore, you must complete this form. If your school is not in the school drop down menu, you will need to choose the option, "Other School - Not an AIA Member" and manually enter the information requested.

Arizona Tax Credits to Support PHS Wrestling

Arizona law allows residents to claim a nonrefundable tax credit by making contributions or paying fees directly to a public school for support of extracurricular activities. The maximum annual credit allowed is $400 for married filing joint filers and $200 for other filers.

When making a tax credit donation online, make sure to select "Wrestling TC Donation" when making your donation.  Make donations at https://az-paradisevalley.intouchreceipting.com or in person at the PHS bookstore.
